We now have set up online questionnaires so that you can let us know important information such as your most recent blood pressure readings.

To access this, first click here to go the log on screen.

 

Then login using your username and password. This will take you to the home screen where you will find a section for questionnaires which you can complete.

You can select the questionnaire which you want to answer:-

Then click submit and the answers will be sent to the practice.
